Do we have your referral?
We are not able to schedule patients for most exams and procedures without a referral from your physician. If you're unsure if we've received your referral, you can call us at 206-292-7734 to confirm.

Insurance questions
We're happy to assist in any way with the insurance process, please visit our Insurance Information page for more information. Note that it is still your responsibility to know what your insurance will cover and where your insurance allows you to have the exam performed. And be sure to bring your insurance card and photo ID with you!

Additional information
  • If you have a referral slip from your physician, please bring it in at the time of your exam.
  • Our staff will remind you of any necessary paperwork that you may need to bring in at the time of your exam.
  • We will relay any precautionary measures or special preparation related to the type of exam.
  • If you need to reschedule your exam, please make sure to call our office so we may schedule you for a later date.
We will call you the day before your exam at the number you provided to remind you of your appointment.
