We accept all major insurance plans and accept the insurer’s approved amount for covered services. While patients are responsible for co-insurance, co-payments, deductibles and non-covered services, we work hard to ensure there are no surprises.

If your insurance company is not on the list, please ask as our participation does change from time to time.

Insurance Tips
We recommend that you review your insurance policy and contact your insurance company before your exam to determine which services are covered by your insurance plan and what information must be provided to your insurance company.

Please bring your insurance identification cards and claim or referral forms with you when you visit our office. We will be happy to answer any questions you may have.

As a courtesy to our patients, we will fill out and submit a standard AMA 1500 insurance claim form to your insurance company. If your insurance plan requires that you make a co-payment, that amount will be collected in the office at the time of your visit. Our office accepts MasterCard, Visa, cash, and personal checks.

For insurance plans not accepted by Seattle Radiologists, you will be responsible for the full charge less any insurance payments received in our office.
